[Bug] Claude makes excessive assumptions and implements unrequested features instead of clarifying intent

Bug Description
Opus 4.8 makes far too many assumptions. I ask a simple question like "will it supports Windows ARM" and it thinks my users will need it and starts implementing it. This is an example but this is true for everything. Trying to do its best is nice when it works autonomously but it is close to not listening to me sometimes
