[Bug] Claude Code not respecting user-defined rules and constraints inconsistently

Bug Description
doesnt matter CLAUDE, Memmory or anything when it wanst it doesnt respects any rules, sometimes i wonder how much you guys cbage models without us knowing.. today its been horrible, listen i start since day one and i am not going anywhere but common, you guys can not make this thing follow simple rules...
