[BUG] Ctrl+V paste from clipboard stops working mid-session

What's Wrong?

Pasting images from clipboard via Ctrl+V works initially but stops working at some point during a session without any error message. Restarting the session fixes the issue temporarily.

Steps to Reproduce

Steps to reproduce:

1. Start a new Claude Code session
2. Copy an image to clipboard (e.g. screenshot)
3. Click in the input field and press Ctrl+V — image pastes correctly
4. Continue working in the same session
5. At some point Ctrl+V stops inserting the image — no feedback, nothing happens

Expected: Image pastes from clipboard consistently throughout the session
