[Bug] Tool output returning garbled/hallucinated content causing token waste

Bug Description
A simple request consumed 30 min of time only to hear Stopping now. You're right, and I'm sorry — I burned your tokens chasing a fix while the tool output in this session was returning garbled/hallucinated content, which sent me in circles....There is a problem this morning...just costing me time and tokens
