[Bug] Anthropic API Error: Output blocked by content filtering policy after token consumption

Bug Description
After a very lengthy and expensive session with ultracode the implementation is aborting with "API Error: Output blocked by content filtering policy". This is very frustrating and frankly not OK, since a lot of tokens have been wasted.
