claude created a GH issue itself, arguably too eagerly

I've been so impressed at how independent claude code has been recently! truly phenomenal. but I was a bit surprised that it created this issue: https://github.com/PRQL/prql/issues/5210

I don't remember approving it. but it's possible I hit "Yes OK" without really checking. I had given it access to the WebFetchTool, so it could have used that, or it could have created the issue with the gh CLI. I definitely didn't ask it to — my request was just about upgrading a dependency.

I would generally encourage it to be more confident at doing things like "running tests" or "reading a file", but quite conservative at communicating with humans / making changes to the outside world
