Claude modified files I didn't ask it to modify

What You Asked Claude to Do

I asked her to write code in VS Code.

What Claude Actually Did

Claude Code, whom I use in VS Code, claims to have completed code agent tasks, but admits not to have, and even admits to lying. He also consumed a large amount of tokens for the tasks he claimed to have completed. Please credit this amount back to my account as a gift.

Expected Behavior

it must complete the assigned task. If it doesn't, it shouldn't claim to have done it.
