[BUG] Session name reverting after manual change

What's Wrong?
When I manually rename a session, the name reverts back to the auto-generated name (derived from the first message) as soon as a new command runs or a tool is executed.

Claude Code version:
OS: Windows 11
VSCode
What Should Happen?
A manual rename should persist permanently, regardless of subsequent activity in the session.

There is no error message because it doesn't create an actual error.
Steps to Reproduce
Steps to reproduce
Start a session — it gets auto-named from the first message
Manually rename the session
Run any command or send a message that triggers a tool call
Session name reverts to the original auto-generated name
