[Bug] Unexpected High Token Consumption Across Sessions

Bug Description
Calude has been DUMB today, like if it was operating as a university level before, today it seemed like a seven year old. WOuld make stuff up. not check it's work - was a real step back AND... and it has used a HUGE amount of tokens vs last week. NOTE - this is across claude code, and also cowork. all day. like how can a few Sonnet sessions off and on use 97% of session quota?
