[BUG] VSCode Extension Description in left side bar not updating

What's Wrong?

What Should Happen?

Despite the description doesnt update in the left bar, the description actually updates in the files. Once the IDE refreshes the title updates. But it doesnt update on the fly, this is the bug

Steps to Reproduce

1. Start a new conversation
2. Change the title in the left side bar
3. Click another conversation
4. 🐞 See how the title you changed reverts into the original one
5. Reload the VSCode software
6. See how the title you change gets back to the title you changed.
