[Bug] Permission prompts hang when tool calls submitted during detailed output mode (ctrl+o)

Bug Description
there's a very frequent bug where permission prompts don't go through. the session just hangs with "waiting..." and I don't see any prompt. I think it happens when the model submits the tool call while I'm looking at "detailed output mode" via ctrl+o. That's the pattern I've noticed, its always happening near a time when I was using ctrl+o.

the workaround is I have to hit esc and ask the model to try again.

you can see it several turns above in this session where I said "ui bug, sorry, plz try again".
