[Bug] Claude Code ignores explicit instructions to avoid parallel tool calls

Bug Description
Claude Code ignored my explicit instruction to avoid parallel tool calls.
Expected:
- Read all feedback/memory files first
- Process files one at a time
- No parallel Read/Write/Edit calls across different files
Actual:
- It admitted it only opened 5 of 19 memory files
- It skipped an important memory file
- It performed multiple parallel Write/tool calls despite being told not to
- Screenshot attached
This caused incorrect workflow behavior and broke user instructions.
