[Bug] Claude Code ignores explicit instructions to avoid parallel tool calls

Bug Description
Claude Code ignored my explicit instruction to avoid parallel tool calls.

Expected:

• Read all feedback/memory files first
• Process files one at a time
• No parallel Read/Write/Edit calls across different files

Actual:

• It admitted it only opened 5 of 19 memory files
• It skipped an important memory file
• It performed multiple parallel Write/tool calls despite being told not to
• Screenshot attached

This caused incorrect workflow behavior and broke user instructions.
