[BUG] Two identical Claude Code icons appear in activity bar after installing a single extension

What's Wrong?
Environment:
- VS Code version: 1.105.0
- OS: Windows
- Claude Code extension version: 2.1.145
Steps to reproduce:
1.Install Claude Code extension from the marketplace
2.Restart VS Code
Actual behavior:
Two identical Claude Code icons show up in the activity bar (left sidebar). Right-clicking confirms both belong to the same extension. Uninstalling the extension removes both icons at once.
What Should Happen?
Expected behavior:
Only one icon should appear in the activity bar.
