[BUG] Claude icon missing in VSCode activity bar (taskbar)

What's Wrong?

The Claude Code icon does not appear in the VSCode activity bar (the vertical icon bar on the far left of the window). Instead, an empty gray box is shown where the icon should be. The extension's main panel (session picker with "New session" and the Local/Web toggle) opens and works normally — only the activity bar icon itself is missing/blank. Reloading the window (Reload Window) does not fix it.

What Should Happen?

fix the prob and let me see the icon :)
