[BUG] Claude Code VS code plugin attached file never gone

What's Wrong?

How to reproduce: upload a file larger than 32MB and ask something, will reply "Request too large (max 32MB). Try with a smaller file." Try with smaller file, the message never gone, same response.

What Should Happen?

Attached file should reset if file size exceeds.
