[BUG] "Accept and auto mode" shows "Permission mode couldn't be changed" warning when accepting a plan (Claude for Mac)

What's Wrong?

App: Claude for Mac v1.7196.0 (2dbd78)
OS: macOS 15 (Darwin 25.4.0)

Actual: Yellow warning banner appears: "Permission mode couldn't be changed. You can try again." Mode still switches to Auto and execution proceeds normally — appears to be a false warning.

Notes: Clicking plain "Accept" and then manually switching to Auto does not trigger the warning. Issue seems specific to the combined "Accept and auto mode" action — likely a race condition between plan acceptance and permission mode switch.

What Should Happen?

Frequency: Reproducible consistently — happens every time I accept a plan with "Accept and auto mode".

Error Messages/Logs

"Permission mode couldn't be changed. You can try again."

Steps to Reproduce

  1. Enter Plan mode
  2. Claude proposes a plan
  3. Click "Accept and auto mode" (⌘↵)
  4. Expected: Plan accepted, mode switches to Auto without errors.
