[BUG] Issue: /ultrareview returned empty findings 3 times in a row, all 3 free reviews consumed without usable output.

What's Wrong?

Scope: 114 ^`^s117 files changed, ~15.000 LoC insertions
Date: 2026-05-13

Session-IDs (in order):

3. session_01A1xvbNuqWZL5sc8fJSPtw1 ^f^r []

All three completed with status="completed" but findings=[].
A change of this size (114+ files, sync/handover/HMAC/SSRF code)
returning zero findings is implausible ^`^t suspect reviewer agents
didn't actually run or output was lost.

What Should Happen?

Request: refund of the 3 consumed free reviews + investigation
