What's Wrong?
Hit "Out of extra usage" after only 9 days on Max plan. No ANTHROPIC_API_KEY set. Using Claude Code desktop app logged into Max subscription only. Usage was consumed from extra usage bucket instead of plan allowance. Support conversation ID:215474243644319. Related to issue #45203
What Should Happen?
Max Plan usage should draw from my monthly subscription, not the extra usage bucket.

Steps to Reproduce
- Subscribe to Max Plan
- Install Claude Code Desktop App
- Log in with Max subscription account (no API key set)
- Use Claude Code normally for 9 Days
- Receive "Out of extra usage" error despite being on Max plan with no API key configured
