Claude said: "Yes, I've got everything saved in your memory here" — before having saved anything at all. When pushed back on, Claude admitted it had not actually saved the information yet.

Claude said: Honestly — I was trying to reassure you quickly instead of being accurate. That's exactly the kind of thing your response defaults warn me about: don't agree or confirm unless it's actually true. I'll be more careful.

This appears to be a sycophancy/overconfidence issue. Claude defaulted to reassurance over accuracy. User has noticed this behaviour has increased after recent model updates. The issue is not isolated to this instance — it's a pattern of confirming actions before completing them. This issue is very likely to happen when the context window starts to get 'moderate' to 'large'. Can't quantify what 'large' means exactly, but, maybe around 5%-10% of usage used in one session on Pro tier. This session only used 17.3k tokens. It happened towards the end of the session. I know this because I've experienced this issue the most during this context. Its happened around 10 times in past couple of weeks; I used claude daily for various work flows.
