Título: Web sessions disappeared after claude auth logout/login

Descripción:
After running claude auth logout and claude auth login to fix a per-user API limit error, all my Web sessions disappeared from both the VS Code extension sidebar and claude.ai/recents. Only one old chat remains visible. The sessions were previously accessible as tabs in VS Code but now appear blank.
