Opus 4.1: Hallucinating Code and Fabricating Incorrect Implementation Details

Bug Description
I am having issue today where the data I have gotten is 100% incorrect. I gave me data where it madeup code that was not there, gave me line numbers that didn't even match the code in the file, gave me features taht were not in the code. Onthe lazy side I have had to ask to check, double check, and still it gives me broken code. It has been really, really bd today at wasting my time simply because it did not do a better walk through the code, even when I hvae asked it to dso. I simply don't know what is going on the Opus 4.1. today. It feels worse to me today than I have experienced in a LONG time. I have never know it to make use such truly incorrect things, and then go off and deicde it should make up features tht I didn't want or ask for... I then have to waste time reverting and all of this wile it is eating up tokens .... lazy == waste of my time and tokens, making stuff up == waste of my time and tokens, not looking deeper when I have asked to == waste of my time and tokens.

This is really, breally bad today and I don't know why.

This is the first time I have ever felt this bad about what is going on... and to the point where I had good trust in Claude and now I am have less and less trust. This simply is not good.
