[BUG] Stale /resume autocomplete entry persists across all project directories

What's Wrong?

When I /resume it autocompletes as /resume Check Aidkido scan visibility in IDE 3 weeks ago · chore/worktree-config · 4.6KB

This happens in all worktrees, and the conversation does not exist in any worktree. Ghost conversation. Conversation can not be resumed. Workaround: don't use tab to autocomplete

What Should Happen?

Autocomplete only to end of /resume and then only longer version on second tab input, and only for existing conversations. (in this scenario, the only existing conversations are in the dropdown list)
