[BUG] "Session title label on prompt frame disappears when interactive prompts (permissions, AskUserQuestion, plan approval) are active"

When there is a question to be answered in claude code CLI, the session name from /rename is overwritten by the horizontal line. Since i run several sessions it's hard to tell which one i'm working on which may influence the answer.

What Should Happen?

The /rename should not be overwritten.

Steps to Reproduce

Do a "/rename" then do anything where it asks you a question. Not sure if this is only the ones with "yes and always" options or the "yes"/"no" only.
