[BUG] Overcharged - Usage Limits Jumps to 100% right away

What's Wrong?
One request in Claude's code, without being completed, causes a usage jump from 0 to 100% and an error message - You've hit your limit. This happens. This happens within a fraction of a second, without Claude processing or even thinking about the request. And after it uses API pricing.

I apologize for the disruption you experienced with your API usage. We take service reliability very seriously, but unfortunately we're unable to issue compensation for degraded service or consumed credits in these situations. - Start Claude Code Session
- Reach the limit normally, and pause Claude's code on user input
- After the limit reset, provide input to Claude's code
Expected Result: Claude Code continues as normal
Actual Result: Claude Code - Causes the 100% usage limit in the account.
