[Bug] Claude interprets system-reminder in tool result output as prompt injection

Bug Description
Claude got really spooked by the fact that <system-reminder> was inside the <output> of a tool result, and assumed it was prompt injection, since it told Claude to "not mention it". That's dumb and really scary for a user...
