[Bug] CLI hangs on clarifying questions in document mode, requires Ctrl+C to refresh

Bug Description
I think I experienced a bug in CC - I was writing a doc (not in plan mode), I asked a clarifying question about the meaning of something and claude code seemed to hang, I think this is UI / CLI issue - as soon as I pressed Cmd+C it refreshed.
I did have a follow up command that was queued but was not worked on, after pressing Cmd+C I got a line from the previous query and my next command started executing.
