[Bug] Claude Code crashes on initialize payload receipt via SSH remote in v2.1.121

Bug Description
ccd-cli 2.1.121 crashes with exit code 1 immediately after receiving initialize payload via SSH remote. Downgrading to 2.1.119 fixes it. Same regression pattern as the 2.1.120 incident from April 25. Server log shows process exits right after WriteStdin with the initialize payload (which contains hooks config).
