[Feature Request] Live theme detection for macOS auto-switching appearance

Bug Description
Theme auto-detection only runs at startup. On macOS with Appearance set to Auto (sunset/sunrise switching), Claude Code stays on the original theme until I quit and relaunch. Would love either (a) live re-detection when the terminal background changes, or (b) a /theme refresh command to re-detect without restarting the session.
