[Bug] Agentic system fails to complete tasks and repeatedly loses MCP connections

Bug Description
So I applied the string of changes to my agentic system yesterday, using up thousands of tokens, and through all of that there was no improvement in this system at all. It continued to fail each test; it continued to drop MCPs and lose connection. I don't know; it just does all these things. It runs all these lines and does multiple adversarial reviews, and then defaults just keep persisting and doesn't really do anything that I've asked it to do or achieve. It's just very disappointing, all in all.
