[BUG] PRO plan 5-hour session consumed after a few md file edits

What's Wrong?

The PRO plan 5-hour session limits reached after 250 lines added and another 258 removed in 10 md files

What Should Happen?

Before cache bugs/effort madness I could implement several phases of a plan for a library with thousands of lines of code and comments

Steps to Reproduce

Tell Claude to edit a file with 100 lines. It will read it, change the 10 lines affected and consumed a relevant fraction of the 5-hour quota

Close to Opus 4.6 launch, from early March usage is random
