[BUG] Ctrl-Z should not exit without warning

What's Wrong?
It is to easy to accidentally exit without warning - particularly after a long session, this can lead to lost context/work.
What Should Happen?
Claude should prompt for exit confirmation, default should be No.

Steps to Reproduce
'Accidentally' press ctrl-z after a 16hr session and see how much you need to repeat yourself to get back where you were!
