[Bug] Claude edits files despite explicit instruction not to and auto-edit disabled

Bug Description
Claude was specifically told not to edit a document that I had made manual edits to. Claude ignored this instruction and somehow made edits while I was away from the computer even though the auto-edit mode was not enabled.
