[BUG] `Cmd+N` constantly opens new chat

What's Wrong?

1. Install https://marketplace.visualstudio.com/items?itemName=anthropic.claude-code.
2. Enable claudeCode.enableNewConversationShortcut.
3. Press Cmd+N multiple times in the claude code panel.
4. New chats are created constantly.

What Should Happen?

Only one new chat should be created.
