[BUG] Plan mode ignored after using new auto mode (Opus 4.7)

What's Wrong?

There are multiple "Plan mode ignored" issues already, but this is new since 2 days.

Plan mode is simply ignored.

This bug occurs in very latest Claude Code using Opus 4.7 at max (xhigh) effort. Already recreated on multiple sessions, occurs almost 100% of times.

What Should Happen?

I should talk with Claude how to plan fixes instead of randomly fixing them in automatic way while still in Plan mode.

Steps to Reproduce

1. Set plan mode.
2. Talk what to do.
3. Agree using "Yes with auto mode"
4. Auto mode finishes task, you discover some bugs after verification.
5. Set plan mode and talk about bugs.
6. Claude Code starts fixing randomly instead of continuing conversation about bugs and plan fixes.
