[Bug] Agent Teams: Sub-agent spawning failures and shutdown handling issues

Question

**Bug Description**
I tried to use the experimental Agent Teams feature. a few months ago it was working great. Recently there are a lot of problems with it. I have to prompt Claude 4 or 5 times before an agent is not spawned as a background/sub-agent. Than sometimes the permissions requests don't make it back to the leader, the agents don't respond back to the team-lead. When the agents where send shutdown requests they didn't always shutdown, I had to manually shut them down or exit the sessio
