[BUG] intermittent sporadic usage policy violation

Bug Description

Steps to Reproduce

2. [Second request - failed]:
3. Same request type works inconsistently but mostly failed. Restarted Claude several times

Expected Behavior

Work as it has been in the past few weeks. I am not the top 5% users

Actual Behavior

Can’t progress with project

Additional Context

Related to issues #2410, #3319, #4104 - appears to be false positive in content filtering.
