[Bug] Plan mode not exiting after accepting all edits in session

Bug Description
When in plan mode, and when accepting an edit (phpStorm mcp enabled) by choosing 'yes, allow all edits in this session', claude code stays in plan mode and keeps asking for permission to change files, while it should exit plan mode and go into 'accept edits on' mode
