[BUG] Claude Code going stupid

I have Opus 4 on the max x20 plan and it's burning only the tokens while breaking code or generating none-working code. it's like working with sonnet 3. (model shows claude-opus-4-20250514).
It started like 2 weeks ago and it's getting worse and worse.
