[Bug] Claude API service degradation - inference quality issues

Bug Description

Claude is essentially unusable even on MAX 20x with OPUS 4.6 and max effort for the past two days.

• fails at basic tasks such as importing Terraform resources
• cheats constantly, pretends to do work
• pushes stuff to production or makes unasked-for changes without prompting and against explicit instructions
• keeps on pushing back on work ("I don't know how to do it. Do it for me")

Essentially, it's impossible to use Claude Code for production work.
