[Bug] Claude repeatedly provides out-of-context suggestions instead of using current session tools

Bug Description
claude code is genuinely unusable, claude consistently gets confused. at one point in this chat, claude told me to go setup an anthropic sdk in a docker container to run data queries instead of doing it within the session
