[Bug] Claude ignores tool use guidelines and manually writes migrations instead of using migration generation tools

Bug Description
claude feels comfortable creating hand-written migrations even defined thousand times to use a tool to generate migrations on database. failure on failure, claude barely respects defined guidelines even each time it says it strengthens the memory file (whatever that may be)
