[Bug] Claude.md rules not enforced: max 3 sentences constraint consistently ignored across sessions

Bug Description
Max plan ($100/mo), sodjacksonville@gmail.com

1. CLAUDE.md rules ignored: "max 3 sentences", "no meta-discussion", "never

summarize what you're about to do" — model consistently violates these across
10+ days, multiple sessions, full reinstalls, config wipes, memory clears.

1. Token consumption: 173 messages, 70% CCU burned in one session with zero

code output. Just conversation and diagnostics.
