[Bug] Permission request system not functional - requests stuck in pending state with no grant mechanism

Bug Description
Claude created a team, then was unable to grant any permissions. Teammates are stuck with "Permission request sent to team "rds-investigation" leader" and the 'main' claud instance has no idea what to do, has no idea about monitoring any inboxes, and no idea how to grant these permission requests
