Chrome extension not connecting to Claude Code CLI

cp__claude-in-chrome__tabs_context_mcp consistently returns "Browser extension is not
connected" despite the extension being installed and active.

Environment:

• Claude Code version: 2.1.97
• OS: macOS 26.3 (Build 25D125), Apple Silicon (ARM64)
• Chrome version: 146.0.7680.178
• Kernel: Darwin 25.3.0

Steps to reproduce:

1. Install Claude for Chrome extension
2. Log into claude.ai with the same account as Claude Code
3. Restart Chrome
4. Attempt browser automation via Claude Code CLI

Expected: Extension connects and tab context is returned
Actual: "Browser extension is not connected" error every time

Notes: Same Claude account confirmed on both CLI and extension. Chrome fully
restarted. Extension visible and active in Chrome toolbar. Claude Code wiped and fresh installed.
