[BUG] /clear command spins ~20 seconds then reloads full conversation instead of clearing

What's Wrong?

Environment:

Claude Code VSCode extension
OS: macOS 25.3.0
Model: claude-sonnet-4-6
Steps to reproduce:

Have an active conversation with substantial context
Type /clear in the input and submit
Observe spinner
Expected: Conversation clears immediately, fresh context starts

Actual: Spinner runs indefinitely, click on prompt history, then the full prior/former conversation reloads — context is not cleared.

Frequency: Reproducible (confirmed multiple attempts in same session)

Additional context: No error message is shown after the spin — the UI silently restores the old conversation, making it appear as though /clear completed successfully when it did not.

What Should Happen?

Claude should clear the context in the current session.

Conduct a conversation with Claude.
Type /clear and activate clear conversation.
Observe the hanging state of Claude attempting to start a new, clear session.
