[Bug] Increased task avoidance and rule non-compliance at medium effort level

Bug Description
I'm seeing way more "that's not my bug" or "simplifying..." and hiding issues as I iterate through code refinement. I'm running at medium effort, and increasing it makes it even worse. basically in my last sessions, the level of scape goating and passing the buck, not doing what needed, ignoring rule files to the "progress" has gone up significantlt recently.
