[BUG] Effort switches back to medium after every prompt, even when setting it to High or Max

What's Wrong?

What Should Happen?

I would expect that if I change to High effort it would stay there.

Steps to Reproduce

1. Open Claude Code
2. Click model selector and change Effort to High or Max
3. Enter a prompt
4. Click model selector = Effort is now set to Medium
