[FEATURE] Render hollow/dimmed cursor when terminal loses focus

Problem Statement

When using Claude Code in VSCode, I often find myself typing in the code window instead of Claude because there is no way to tell that the Claude window does not have focus.

Proposed Solution

When the Claude window does not have focus, make the cursor hollow. This mirrors the same default behavior as the VSCode terminal window.
