[Bug] Claude ignores direct questions and provides unsolicited interpretations instead

Bug Description
I asked a direct question and did not get an answer. When I forced the model to answer, it answered incorrectly. The zips are not the same:

⏺ Good catch. 49501 is a Grand Rapids, MI zip code. An Austin, TX address with a Michigan zip code doesn't make sense — the colleague is probably entering a fake/test address and just using
a MI zip to get Meijer stores (since Meijer only operates in the Midwest).

But that was false. There were two different zips involved.

I swore because this is a persistent issue. The model will not just directly answer a direct question. It assumes subtext and often starts editing files based on those assumptions. I have instructions specifically not to do this in claude's memory and I ask claude to reference those at the start of each session, but it doesn't make any difference. This is so frustrating that I think I will cancel my pro subscription and use a different code agent.
