[Bug] Ultraplan: Comments ignored, misplaced, lost between CLI/UI transitions

**Bug Description**
ultraplan is the most annoying feature I ever had. I just used it. my claude code in the cli submitted a plan, ultraplan starts planning, my comments got mostly ignored, comments got placed on wrong positions (probably some UI bug - I just the Mac app), comments vanished randomly - after 5-6 rounds my comments (8 comments in total) finally got adressed. "teleporting" the session back to my cli led to another planning round in my cli (why?) and basically everything I worked ou
